Question: What causes double vision after injection?

I went to have a dental implant today (2nd attempt as the 1st one failed to integrate). Almost immediately when the doctor injected the anesthetic, my left eye started to burn. It felt like it was going to explode. My vision got very blurry and then I suffered double vision which lasted about 25 minutes. When I covered my left eye, everything was normal. When I covered my right eye, no double vision but everything was at a tilt. The dental assistant seemed scared and the dentist left the room after I asked for a few minutes before proceeding. The assistant also seemed concerned about the "white splotches" on my face which the doctor said was a reaction to epinepherine in my sinuses. I seem to be fine now, but would like to know what happened and If I should be concerned. I`ve had lots of dental work and lots of injections and outside of a sugar rush have never had anything like this happen before. Thanks.

Answer: What causes double vision after injection?

Thank you for you question. I presume this was the back of the upper jaw that was anesthetized. This is a recognized complication, although infrequent. The symptoms usually resolve in the time frame for the local anesthetic to wear off, 1 – 6 hours depending on the anesthetic involved.

The eye symptoms are likely due to fibers of the sympathetic nervous system becoming anesthetized and drooping of the eyelid and possibly flushing of the eye would also accompany your symptom complex. The white splotches are the result of the epinephrine constricting the blood vessels in that area. Since the face gets so much collateral (extra) blood supply to most areas, this almost never results in any permanent damage.

Occular complications following dental local anesthesia

Abstract

Objective: To determine the frequency of appearance and the factors most commonly associated with ocular com-
plications following dental local anesthesia, also establishing the location and type of anesthesia used.
Study Design: An indexed search in the Pubmed and Compludoc databases was carried out with the keywords
“oral anesthesia”, “ocular”, “ophthalmologic”, “damage”, “complications”, “injection”. We established a limitation
that the literature had to have been published after the year 1970. A total of 19 articles were obtained, forming a
total sample of 37 patients. The patient’s sex, age, nerve anesthetized, type of anesthetic used, ophthalmological
complication present, recovery time, treatment and side effects were analyzed.
Results: There is a higher involvement of females (77%). The average age was 34.2 years. There was no preference
for an anesthetic technique. Diplopia was the most common complication (65%), which coincides with the data
from other authors. Almost all of the complications were of a temporary nature, with an average recovery time of
68 minutes.
Conclusions: This is one of the few studies of its kind in dental literature, it thus being difficult to make precise
conclusions. Ophthalmological complications are seldom a problem, diplopia being the most common among
them. The authors appear to indicate an intravascular injection of the anesthetic as the cause of the problem, and
therefore, it should be avoided in order to prevent accidents at the ocular level.

Researchers from Case Western Reserve University School of Dental Medicine and University Hospitals Case Medical Center statement initial results from a minute example that inflammation from gum disease plus prostate problems just might be linked. Oral thrush (oral candidiasis) is a condition in which the fungus Candida albicans causes an infection on the lining of the mouth. It is also known as "Thrush". When occurring in the mouth or throat of adults it may also be termed candidosis or moniliasis.

Oral thrush causes white lesions, usually on the tongue or inner cheeks. The lesions can be painful and may bleed slightly when they are scraped. The infected mucosa of the mouth may appear inflamed and red. Sometimes the condition may spread to the roof of the mouth, gums, tonsils or the back of the throat. It is also possible to get thrush in other parts of the body, such as the vagina, nappy area or nail folds.

According to Medilexicon's medical dictionary, oral thrush is:

"Infection of the oral tissues with Candida albicans; often an opportunistic infection in humans with AIDS or those suffering from other conditions that depress the immune system; also common in normal infants who have been treated with antibiotics."

For healthy people, oral thrush is a minor problem. But it may be more severe and difficult to control for people who have a weakened immune system.

Oral thrush most commonly affects people who wear dentures. People who have difficulty keeping their mouth clean, people with diabetes and those who take steroids are also at a higher risk of developing the condition.

Some antibiotics may cause thrush, especially if taken over a long period. A dry mouth caused by certain drugs, or after chemotherapy or radiotherapy to the head and neck, may also lead to thrush. Very rarely, oral thrush is one of the early signs of HIV.

What are the causes of oral thrush?

Oral thrush and other candida infections can occur when the immune system is weakened by disease or drugs such as prednisone, or when antibiotics disturb the natural balance of microorganisms in the body.

The immune system works to repel harmful invading organisms, such as viruses, bacteria and fungi, while maintaining a balance between "good" and "bad" microbes that normally inhabit the body. When these protective mechanisms fail, an oral thrush infection can start.

Thrush infections are caused by an increase in the levels of fungus (yeast) naturally present in the mouth. Yeasts may grow and cause symptoms if:

* the immune system is weakened
* the healthy bacteria normally found in the mouth are killed
* very little saliva is produced
* there are changes in the acidity of the mouth

These illnesses increase the risk of oral thrush infection:

* HIV/AIDS. The human immunodeficiency virus (HIV) damages or destroys cells of the immune system. It increases the risk of opportunistic infections that the body would normally resist. Repeated bouts of oral thrush may be the first sign of an HIV infection.
* Cancer. The immune system is likely to be weakened both from the disease and from treatments such as chemotherapy and radiation, increasing the risk of candida infections such as oral thrush.
* Diabetes mellitus. In people who do not know they have diabetes or if the disease is not well controlled, the saliva may contain large amounts of sugar, which encourages the growth of candida.
* Vaginal yeast infections. Vaginal yeast infections are caused by the same fungus that causes oral thrush. Although a yeast infection is not dangerous, pregnant women can pass the fungus to the baby during delivery. As a result, the newborn may develop oral thrush.

Signs and symptoms of oral thrush

A symptom is something the patient feels and reports, while a sign is something other people, such as the doctor detect. For example, pain may be a symptom while a rash may be a sign.

Initially, oral thrush symptoms may not be noticeable. Many mild oral thrush infections are painless, although the condition can sometimes become quite sore.

Signs and symptoms may develop suddenly, but they may persist for a long time and can include:

* A cottony feeling in the mouth
* Cracking at the corners of your mouth
* Creamy, white lesions on the tongue, inner cheeks and sometimes on the roof of the mouth, gums and tonsils
* Lesions with a cottage cheese-like appearance
* Loss of taste
* Pain
* Slight bleeding if the lesions are rubbed or scraped

People who wear dentures, and have them taken out, may be able to see a very red area where the dentures have been.

In severe cases, the lesions may spread downward into the esophagus. This may cause difficulty swallowing or an uncomfortable feeling as if food is getting stuck in the throat.

Infants and breast-feeding mothers

Infants may develop the distinctive white mouth lesions. They may have trouble feeding or be fussy and irritable. During breast-feeding, babies can also pass the infection to their mothers. The infection may then pass back and forth between mother's breasts and baby's mouth. Women whose breasts are infected with candida may experience the following signs and symptoms:

* Shiny or flaky skin on the areola
* Stabbing pains deep within the breast
* Unusual pain during nursing or painful nipples between feedings
* Unusually red, sensitive or itchy nipples

Seek medical care if those signs and symptoms appear. In older children, an underlying condition such as diabetes may be the cause.

Dental Visits

The ADA recommends regular dental check-ups, including a visit to the dentist within six months of the eruption of the first tooth, and no later than the child's first birthday. Preventive care such as cleanings and fluoride treatment provide your child with "smile" insurance. Routine dental exams uncover problems that can be treated in the early stages, when damage is minimal and restorations may be small. When necessary, X-rays are taken to see how the teeth are developing and to spot hidden decay.

Early Childhood Caries (Baby Bottle Tooth Decay)

Baby bottle tooth decay can destroy your child's teeth. It occurs when a child is frequently exposed to sugary liquids such as milk, including breast milk, fruit juice and other sweet liquids. The ADA recommends the following steps to prevent your child from getting baby bottle tooth decay.

-- Begin clearing your baby's mouth during the first few days after birth. After every feeding, wipe the baby's gums with a damp washcloth or gauze pad to remove plaque.

-- Never allow your child to nurse or breast feed for prolonged periods and don't give him or her a bottle with milk, formula, sugar water or fruit juice during naps or at night in bed.

-- Encourage children to drink from a cup by their first birthday.

-- Help your child develop good eating habits early and choose sensible, nutritious snacks.

Mouth Protectors

Any child involved in a recreational activity, such as soccer, hockey, football, roller blading, riding a scooter and even bicycling should wear a mouth protector. There are "stock" mouth protectors available in stores and a better-fitting variety, which are custom fitted by your dentist. Ask your dentist about using a mouth protector.

Dental Emergencies

Knowing how to handle your child's dental emergency can mean the difference between saving or losing a tooth. The ADA recommends the following tips on what to do for your child in case of:

Knocked-Out Tooth: Hold the tooth by the crown and rinse off the root of the tooth in water if it's dirty. Do not scrub it or remove any attached tissue fragments. If possible, gently insert and hold the tooth in its socket. If that isn't possible, put the tooth in a cup of milk and get to the dentist as quickly as possible. Remember to take the tooth with you!

Toothache: Rinse your mouth with warm water to clean it out. Gently use dental floss or an interdental cleaner to ensure that there is no food or other debris caught between the teeth. Never put aspirin or any other painkiller against the gums near the aching tooth because it may burn the gum tissue. If the pain persists, contact your dentist.

Bitten Lip or Tongue: Clean the area gently with a cloth and apply cold compresses to reduce any swelling. If the bleeding doesn't stop, go to a hospital emergency room immediately.

Well, after trying the electronic toothbrush, I can tell my teeth are much cleaner. I like the digital meter that shows me how long I should brush each section of my mouth. I guess I was not brushing long enough before. This is my husbands second electronic brush from Oral-b, but he says this one is a much better model than his previous one. He likes the flossing brush, while I like the whitening brush. I actually have not tried the flossing brush yet, but I will soon. The battery lasts a long time, so no need to recharge every day. So, one charger is good enough. We took it for a long weekend and it was fine the whole trip. I would recommend this to my friends.

Oral B Professional Care SmartSeries 5000

Our dentist recommended the Oral B 5000, because, she said, "it's the best consumer device for removing plaque, overall cleaning, and cleaning between teeth".

Now that we've used it for a couple of weeks, I'd have to say she was correct. It does a much better job cleaning your teeth than a manual toothbrush. Your teeth won't feel like they've just been professionally cleaned, but pretty close.

Oral B includes a mini-CD to explain the correct usage. You'll also receive a timer that reminds you when to move to a different quadrant of your mouth; a useless gimmick for an adult, in my opinion, but it might encourage a child to conscientiously brush their teeth. The timer is about the size of a pack of cigarettes, sits anywhere on a counter-top or sticks to a wall with the included double-sided tape, is operated by two AA batteries, and wirelessly communicates with the toothbrush. The timer's LCD screen also indicates which brushing mode an operating toothbrush is set to: daily clean, sensitive, massage, whitening or deep clean. This feature is useful for learning how to use the toothbrush; i.e., by you recognizing the sound of each brushing mode. When the toothbrush is off, the timer is a digital clock, which is useful.

Oral-B makes several different types of toothbrush heads: Daily Clean, Sensitive, Massage, Whitening, Ortho Care, Deep Clean, Power Tip and Floss Action. All of these work fine, and none of them are cheap.

If you apply "too much" pressure while brushing, the oscillating movement of the brush head continues but the pulsation will stop, the feeling will change, and the sound will be different. The normal brushing speed & pulsation is somewhat more aggressive than I would prefer, but it does the job. The operating sound is quite minimal, not objectionable at all.

The toothbrush handle diameter is a bit large to hold comfortably and also because of its length and diameter, a little unwieldy when brushing the back teeth. (Of course, no electric toothbrush is going to be as small as manual brush.) The handle has a slip-proof coating on the back. The device is probably too big for small children to use comfortably.

The base assembly is designed to hold the battery-powered toothbrush upright and the various toothbrush heads in a covered tray. The base accepts the snap-in battery charging ring. That ring refreshes a completely discharged toothbrush in several hours. The instructions recommend leaving the charging ring plugged in so the toothbrush is always fully charged and ready for use. There are no exposed electrical contacts on the charging ring, to prevent shocks in case it slips into the sink. The toothbrush also has no metal contacts, so it's a mystery to me how electricity flows from the charging ring, but it works! The base assembly has four depressions with a removable lid to hold that many brush heads. The base assembly (except the removable charging ring) can be cleaned in the dishwasher.

I can't recommend this enough. This toothbrush made my teeth so much whiter and each time left my mouth feeling so clean, like after you come from the dentist after cleaning. It's amazing, and I love the smartguide. It really helps you focus on where to brush and alerts you if you brush too hard. I like the fact that they have different kind of heads, like floss action, interdental, sensitive or the whitening head (love it!). I used to have a sonicare, this blows that away. Highly recommend.

The missing items are one of the three promised brush heads and a charging station with brush head storage. There is a charging station included but it has no provision for storing brush heads. I consider the brush head storage to be a critical omission; the missing brush head not so critical but still less than promised when ordered.

I have searched the Amazon site and cannot find any way to contact them about these omitted items. I could cancel the entire order but there should be some means for contacting them to report these omission and allow them to correct the situation without reversing the entire order. I think that Amazon should prominently place a "Contact" link through which problems of this sort can be reported either online or by phone.

I can't legitimately rate the item because I haven't used it yet but Amazon demands that a rating be given in order to provide a review.

I just purchased this oral b pro 4000. Amazon says its supposed to come with 3 brush heads but it only came with 2 (floss, whitening), supposed to come with a cover for the brush but none was included. Travel case is included though. I am very disappointed. After searching for the perfect one, I chose this one cuz it had everything I was looking for...3 brush heads, travel case, cover for brush head. I haven't had a chance to use it yet since I just got it. Hopefully it won't be disappointing. I did call Amazon back and got a $10 credit.

Crest Whitestrips actually work very well, but this version has several issues which would prevent me from buying them again.
Unlike Crest Whitestrips I've used in the past, these are very flimsy and hard to handle and to apply to the teeth. One on, they are too slippery and flimsy to stay in place well.
The heavy plastic container them came in is absurd. Why do we need a thick plastic compartment that weights 15 times more than the actual product? Am I going to save it to keep my valuables in? What was wrong with the simple paper box the other version of Whitestrips used?

I've decided to try these again after using them several years ago. They did whiten my teeth, but I also remembered they caused my teeth to be super sensitive. My teeth do not have a tendency of being sensitive- unless I try to bite into ice cream; I cannot do that without feeling it. Before I put even the first day's strips on my teeth, I made sure to purchase some Sensodyne toothpaste, and I am so glad that I did. I followed the directions and put the strips on (without brushing first) and within the 30 minutes of applying them my teeth started getting painfully sensitive. I had such acute pain in my teeth a couple of times within those 30 minutes. Several hours later my teeth still feel very sensitive, and it is without any exposure to heat/cold. These will whiten your teeth, but you will definitely feel it.

Sometimes the strips will get out of place due to the saliva that is produced by having them on. I think the price should be about $15. what bothers me most is that the part of my teeth closer to my gums look less white..eventhough the strips touched my gums too and caused pain there.

Purchase of the well-reviewed electric toothbrush has been frustrated by inconsistent information on the Amazon.com website regarding the number of brushheads included. The product info says '3' and the image says '2'. I called Amazon trying to get 3 brushheads to test out prior to purchasing a follow-on set. Long story short - Amazon said to call Oral-B. Oral-B had no idea and said to call Amazon. Apparently Amazon custom packages these as do other companies and making the order became a gamble. I was unlucky, then, because although the website states 3 brushheads I received 2 as per the image on the box. To me this is unsatisfactory, especially when Amazon customer service said the mismatched info would be corrected in days, which it was not. That said, in general there appear to be several versions of the SmartSeries 5000 toothbrush with varying brushhead counts, up to 4 or more. And this confusion is added to by the fact that these are rebranded 'Triumph'-series toothbrushes, now called SmartSeries 5000s.

To the point: the initial experience upon receiving the product was that it was an unfinished beta-product of sorts. My sense was that the toothbrush holder could be wall mounted. This is not the case. So it now is perched inside my medicine cabinet on the lowest shelf, and I worry about the toothbrush falling out onto the sink. The holder is not well designed to prevent such occurrence. The wireless display is agreed to be surprisingly useful, though is impractical in use as the best place to use the affixing pad is dead-center on the bathroom mirror. An angled plastic bracket that has a rotating joint should allow positioning of the device off the wall and at various angles so to be seen. Instead, I keep it inside the medicine cabinet and take it out and lean it against my electric shaver so it faces upward from atop the sink, only when using the toothbrush. Such an additional mounting clip/attachment should also allow angled placement on a flat surface and not just a wall. So that a 30-degree tilt (+/-) could angle the display when placed atop the sink.

Using the toothbrush: I could not stand it on the first run. It was like having a floor polisher in your mouth. If you have a battered nervous system it may only be possible to use it on the 'sensitive setting'. The problem as mentioned elsewhere in these reviews is that you have to cycle through several maximal settings to get to the sensitive, more gentle setting. And there is no memory on the device (which is why it seems to be a beta-product or unrefined at this most basic level of efficient use). This was surprising and for this reason alone I would avoid purchasing this product until a new revised version fixes such basic oversights. That is work for Oral-B. Yet I also would wait until Amazon fixes their information and stops selling the product as including 3 brushheads when it ships with 2. I do not understand how this misrepresentation is acceptable as a business practice, especially when multiple calls to remedy the situation do nothing to change it, even after assurances by Amazon that it is being addressed.

All in all I would wait until the next version because 100+ dollars for such a product is not a good investment in its present design incarnation. The next product perhaps will fix these and other issues (such as default setting, wall mountable toothbrush holder, cap for toothbrush head, tilt-adjustable/angled attachment for wireless display, accelerometer for gesture control of settings, 5 second delay when turning on the device so to get it inside your mouth prior to floor polisher modes, and perhaps reworking the entire user interface, especially the useless LCD GUI on the handle. e.g. if the LCD is only used for battery-indicator status when off, why not replace this large screen display with a single color e-ink strip or other display tech that shows charge amount as a linear graph, e.g. 2/3rds battery full). Also, these products should be selling at half their retail price for what they are.

In general you will be a beta-tester of a product that never should have shipped in its current substandard condition of functioning. Perhaps it is indicative of corporate malaise. It is thus bothersome to think that when a fix arrives that what should have been fixed can only gained via repurchasing. For this reason I think an upgrade path should be given to those who invested in this product, and are left disappointed yet still need and find use for it. If it were available I would be interested in upgrading for a nominal fee from the current product to the next product if such a trade-in program were available. That would be the right thing to do for selling such a subpar device versus a quality product, i.e. while useful it includes and relies on needless ongoing frustrations.

My recommendation would also be that Oral-B standardizes their packaging and ships a single product under the 5000 label, or whatever the next version will be, and does not have a dozen different competing packaging strategies which confuse and complexify the product offering for potential users. While I like the idea of the product its exuberant praise to me seems oversold. It is okay, it is not great. It is basically the same technology that has been around for 30+ years now and seemingly has not advanced significantly. And it should and could be much better. Ideally a standardized package would include a variety of brushheads to test out, such as a sampler pack (5 types, say), and also an instructional DVD (included in some sets) which is an item of no-cost to include.

In summary, it is a letdown to be excited about investing in an electric toothbrush for all the benefits it can offer and then to be disappointed in the process of finding out basic information (which is wrong/inaccurate) and then having a different product shipped, which then also is not due the high praise given it, which only reinforces the continuation of such mediocrity. Three stars is a fair assessment. One star for Amazon and it's customer service for dealing with inquiries and not responding to conflicting information even after stating they would. Two stars for Oral-B for not having a better product on the market which addresses basic design issues and known flaws and concerns. Now, if only there were an upgrade path... There is no good reason a 5 star product should not be shipping in place of this 3 star product. Oral-b is not investing in the product itself, only exploiting what can be gained from the last substandard version by rebranding it. This is a foolish strategy that dilutes the integrity of the brand which then is reflected in the poor design quality, needless confusion, and the unsatisfactory customer experience throughout the entire process.

Maybe a health-technology startup will challenge the malaise and rightfully steal the market away from the behemoth non-innovators and wake things up again. Until then there is little or no choice, which is an indication of the status quo. Its what being sold yet its not good enough to buy, a prisoners dilemma.

But the design of this thing is amazingly retarded for a $100 toothbrush. For starters, no wall mount. So, if you have a pedestal sink or limited counter space, don't bother with this product. If you have about a square foot of bathroom counter space to devote to this clumsy beast, go for it, it cleans well. Also, if you don't mind a gigantic protruding eye-sore of a power adapter, this product is also for you.

Don't get me wrong, it cleans teeth. It's just sad that the accessories are so poorly designed.

I have no cavities and was happy with my Sonicare toothbrush which I had been using for several years. On my recent dental check-up, my dentist noticed that my teeth were stained from the tea that I drink and asked me what type of toothbrush I use. She recommended that I consider buying this Oral-B toothbrush because it could help remove the tea stains.

This is an excellent electric toothbrush and it makes my teeth feel smooth and clean after brushing. The timer and pressure warning feature are nice and the brush is much easier to keep clean than the Sonicare I used to use. The only negative comment I can think of is that the brush head might be a little big for some people; I found brushing around my back molars a little difficult. Overall, though, this German-made toothbrush is probably one of the best electric toothbrushes available and I would highly recommend it to anyone looking for an electric toothbrush.

These brush heads have replaced the original heads for Advance handles. I've been using them for about a year and have found a few pros and cons:

Pro:
o They are smaller, lighter and more flexible. Easier to use and fit in my mouth better.
o Seem to last longer than original Advance brushes.
o Clean teeth a bit better than original brushes.

Con:
o Don't fit as well on the handle, sometimes need to be tightened a few times.
o Brush feels a bit harder on gums than original Advance brushes.

Overall, a good replacement brush head to keep my four year old Advance handle working well. And Amazon's Subscribe and Save program is a great way to get them regularly at a good price.

The only complaint I have with the brushes is that they don't always prevent moisture from seeping into the screw-on assembly part of the brush. That results in mildew and goop building up in the base of the brush assembly even after rinsing. It basically makes cleaning a bit harder.

The annoying clamshell packaging does not have any perforations to open it easily so you must use a box cutter to open it which is somewhat irritating.

I have used this product for reduction in cold sensitivity caused by gum recession and exposed roots. I have found Crest Sensitivity to be much more effective than the competing brand Sensodyne. This result is unexpected since both Sensodyne and Crest Sensitivity have the same active ingredient, namely potassium nitrate in a concentration of 5%. Why one product would work better or worse than the other is completely a mystery to me, but the bottom line is that Crest Sensitivity seems to work.

I can't find enough of this stuff... It's either becoming very popular or is very under stocked, since I find myself cleaning out the entire stock of the two stores nearest me on a weekly basis.
You should be able to find this for 99 cents.... beware, some places charge significantly more.
Great for diabetics like me, and very refreshing after a swim or hockey game. I like all the flavors except the red colored ones, acai, and yum berry, which remind me too much of hawaiian punch.
I have a lot of food allergies too, but haven't had any problem with any of the 0 calorie lifewaters.
I'm a natural skeptic and had to look up the ingredients before adopting something that seemed too good to be true. The sweetener used in this stuff is called Erythritol, which from what I've read is a natural substance from fruit and doesn't have all the side effects other sweeteners do, like promoting cavities, obesity, diabetes, or diarrhea.
I took it easy at first because i have sensitive bowels, but have found that i can drink this stuff from morning to night and not have any trouble. Although as with anything I eventually get bored and have to switch back to ice tea for a while.
I can't say the same for the regular lifewaters or any other beverage except unsweetened ice tea, and water, which get a little boring after a while.
This stuff has helped me kick the soda habit and reduce my appetite... thank you sobe!
It would be wonderful if I could have this stuff delivered by the case instead of having to go hunting for it a few bottles at a time.

Researchers have uncovered the complete genetic make-up of the cavity-causing bacterium Bifidobacterium dentium Bd1, revealing the genetic adaptations that allow this microorganism to live and cause decay in the human oral cavity. The study, led by Marco Ventura's Probiogenomics laboratory at the University of Parma, and Prof. Douwe van Sinderen and Dr Paul O'Toole of the Alimentary Pharmabiotic Centre at University College Cork, is published December 24 in the open-access journal PLoS Genetics.

Bifidobacteria, largely known as long-term beneficial gut bacteria, are often included as probiotic components of food to aid digestion and boost the immune system. However, not all species within the genus Bifidobacterium provide beneficial effects to the host's health. In fact, the Bifidobacterium dentium species is an opportunistic pathogen since it has been linked to the development of tooth decay. The genome sequence of B. dentium Bd1 reveals how this microorganism has adapted to the oral environment through specialized nutrient acquisition features, acid tolerance, defences against antimicrobial substances and other gene products that increase fitness and competitiveness within the oral niche.

This report identifies, through various genomic approaches, specific adaptations of a Bifidobacterium taxon to a lifestyle as a tooth decay-causing bacterium. The data in this study indicate that the genome of this opportunistic pathogen has evolved through only a small number of horizontal gene acquisition events, highlighting the narrow boundary that separates bacteria that are long-term residents on or in the human body from opportunistic pathogens.

We have described two patients with ipsilateral optic neuropathy and one patient with an ipsilateral abduction deficit and a dilated pupil occurring immediately after dental procedures involving the upper jaw. In one patient, the optic neuropathy did not recover; in the other, it recovered completely within 10 days. The single patient with an ocular motor disturbance recovered completely within 1 day.

Thirty-nine cases of ophthalmic complications resulting from dental anesthesia have been published in the English literature since 1960, most of them in dental journals. Thirty-six reports have described transient manifestations that disappeared within 5 hours of administration of the anesthetic. Because of the usually benign and transient nature of the manifestations, most patients were never examined by an ophthalmologist. Therefore, the nature of the deficits is not well documented.

Of the 36 reported cases, 23 have occurred after upper jaw anesthesia (posterior superior or a middle superior alveolar block) have occurred after lower jaw anesthesia (mandibular block) . The commonest symptom has been diplopia, mostly secondary to lateral rectus palsy. Other manifestations have been visual loss, ptosis, mydriasis, and Horner syndrome. The exact mechanism by which the anesthetic causes the deficits remains unsettled.

In upper jaw anesthesia, the anesthetic is believed to cause neuro-ophthalmic manifestations by any of the following mechanisms:

1. Simple diffusion from the pterygomaxillary fossa to the orbit through defects in the bone or via the vascular, lymphatic, and venous networks that link these spaces .

2. Inadvertent injection into the orbit through the inferior orbital fissure.

3. Inadvertent intra-arterial injection into the superior alveolar artery with retrograde flow to the internal maxillary artery and then to the middle meningeal artery. A middle meningeal branch occasionally penetrates the superior orbital fissure and anastomoses with the lacrimal branch of the ophthalmic artery. In support of this theory is the observation that blanching and anesthesia of the skin of the lateral upper and lower eyelids, supplied by the lacrimal nerve and artery, are sometimes described in conjunction with transient ophthalmic manifestations. The risk of penetrating an arterial lumen increases when using a non-aspirating syringe and injecting rapidly under pressure.

4. Inadvertent venous injection into the pterygoid venous plexus. From there the solution can reach the orbit through the cavernous sinus which receives drainage from the pterygoid venous plexus via emissary veins through the foramen ovale and drainage from the orbit via the inferior and superior ophthalmic veins. The pterygoid venous plexus also communicates with the inferior ophthalmic vein through the inferior orbital fissure .

5. Inadvertent scraping of the wall of an artery. The trauma sets up a sympathetic impulse that travels from the anterior, middle, or posterior superior alveolar arteries back to the internal carotid plexus and from there through the ophthalmic artery to the orbit. Decreased sympathetic activity would then produce vasoconstriction caused by the unopposed parasympathetic tone that in turn would cause ischemic deficits .

The mechanism of transient injury after lower jaw anesthesia can hardly be explained by simple diffusion of the anesthetic solution, because the injection site is too far from the orbit. Instead, the proposed mechanism is inadvertent intra-arterial injection. Because the inferior alveolar artery, a branch of the internal maxillary artery, lies adjacent to the alveolar nerve, an accidental intravascular injection is possible. In fact, the chances of penetrating the inferior alveolar artery during mandibular block are much higher than penetrating the posterior or middle superior alveolar arteries during upper teeth anesthesia, not only because of its proximity to the alveolar nerve, but because its lumen is relatively large.

The mechanism of Horner syndrome after inferior alveolar nerve anesthesia remains difficult to explain. It is proposed that inadvertent cervical sympathetic block results from a misdirected injection into the pterygomandibular space. From there the anesthetic would reach the pre-vertebral space and cause transient sympathetic chemical denervation .

Three cases have been reported in which ophthalmic complications were delayed, prolonged, or permanent and thus cannot be easily explained by a direct effect of the anesthetic drug (1-3). Hyams (1) reported a young woman who fainted immediately after the injection. The next day she noticed diplopia. Paresis of third and fourth cranial nerves was diagnosed. Tomazzoli-Gerosa et al. (2) described a young woman who experienced complete hemifacial sensory and motor paralysis immediately after an inferior alveolar nerve block. Several hours later, she lost vision in the ipsilateral eye. Two months later, optic disc pallor was observed, phenomena that resemble our case 2. Another unusual report (3) was that of a 14-year-old girl who complained of blurred/double vision and occasional light flashes in her OS 4 hours after the dental treatment was completed. The symptoms persisted the next morning and finally disappeared approximately 24 hours after the injection. No explanation is given.

Permanent loss of vision has been reported after anesthetic injections in other parts of the face, particularly during rhinosurgical procedures . In these cases, visual loss has been attributed to the vasospastic effect of adrenaline. This supposition is supported by the finding that ophthalmic artery pulse pressure is reduced by 50% after retrobulbar injections of Xylocaine and adrenaline.

In our case 1, diffusion of the anesthetic to the orbit or cavernous sinus alone could have explained the ophthalmic manifestations because they disappeared as the anesthetic effect wore off. Our case 2 experienced a permanent loss of vision with a course that resembled anterior ischemic optic neuropathy. We believe that the vasospastic effect of the adrenaline may have triggered this process. We cannot entirely rule out a coincidence, inasmuch as the same condition occurred in the fellow eye 1 year later without an identifiable trigger.

Our case 3 is difficult to explain because the period of optic neuropathy lasted longer than the anesthetic effect. It is possible that the neuropathy was secondary to the effect of lidocaine. Animal studies have demonstrated a possible toxic effect of lidocaine on rat retinal ganglion cells .

Q2 Most painful to wear braces? : I had my braces on for over 3 years and they were recently taken off. The pain is something you get used to, but if it is really bothering you, try taking ibuprofen, it helps. For me, it took about a week to two weeks (depending on what was done in the office) for the pain to stop. I ate a lot of pudding, jello, soup, mashed potatoes, anything soft. I am not a vegetarian, but just try looking at what is in your diet that is really soft. If you can swallow it without chewing, it is fine to eat when you are in pain, but watch the temperature, sometimes extreme temps can cause discomfort.

HOW MUCH WILL I HURT? IS IT PAINFUL TO WEAR BRACES?

“Not too much” and “so so” would be the 2 fast answers. Before you just renounce the idea of having straight teeth, let me try to explain this better. There are millions of people who treat their crooked teeth (many kids or teenagers), it shouldn’t be so painful. And it’s not.

I can tell you about my own experience in 2 systems: Damon (metal braces) and Invisalign. I got my metal braces 1 year ago. The entire process was scarier when reading about it, than the real thing. The braces got on my lateral teeth (we only braced these) on both arches and I was ready to go. With my dental wax too and the new life ahead of me. I was told to use Kethonal or any other painkiller if I couldn’t cope with the pain, prepare for few hard days and then it will be better. And it was.

After 2-3 hours I was already feeling some pressure on my teeth. It was something like my finger was in my mouth and I was pushing them a bit. Nothing too serious, sure, I didn’t take any medication. I ate with care and then went to sleep.

Morning came: I got up and accidentally clenched my teeth. AUCH! The pain almost made me scream. After getting some nice swearing done (that was the least I could do), I got to the kitchen to eat something. Yeah, sure. I wasn’t able to press my teeth against anything since it was painful. I didn’t hurt if they didn’t touch anything (so I just had my mouth slightly open as to have the teeth not touch). The pressure was still there, but the pain would appear ONLY when I clenched my teeth. So, I stopped doing this and didn’t take medication at all either. It really wasn’t the case.

I started eating mashed potatoes and all kinds of soft foods. For 2-3 days I was just like an old woman, even my grandma ate better (and you can imagine she didn’t miss the chance to observer how “old” I was).

In few days the pain wasn’t that big anymore and I started eating bread (not anything hard) and some foods that weren’t as soft as a newly born’s menu anymore. I was able to clench my teeth without kicking anyone and my brushing was more pleasant. I did try to keep a perfect hygene in the pain days and it was pretty nice.

After 1 week I was able to ditch the wax too (for few days I made my fill with dental wax and was certain I’d need some special deliveries, maybe a truck of wax, due to the huge need I had) and eat like a normal person. My doctor told me it would be a good idea to not try anything too hard (nuts or something like that) so that I don’t damage my braces, but God is my witness I could eat if I wanted to.

After each adjustment I’d have 1-2 days of some pain (not huge one, never took medication for this) and then it was all OK. The first days are the worse (when you get braced in), afterwards your teeth get adjusted to the braces so that you’d be able to “recover” faster from each adjustment.

Now I have Invisalign and still expect that pain. Maybe I got used to the metal braces that are a bit more “strong” or maybe I am just lucky. After getting the Invisalign trays on I got home and started eating fast (so that I can then eat soft foods again because of the possible pain). Waited for some hours, no pain. Just a slight pressure on my teeth. Next day: one incisor hurt. “Thank God, I’m normal.” Nothing else. In few hours the incisor stopped hurting while I was able to eat normally, even if I was prepared for another “old woman menu” for some days at least.

1 week into the treatment: I really doubt I’ll see pain anymore. I still feel that slight pressure on the teeth (more like a tickle) and nothing more. Looks like my life now will be pain-free. So .. the painkillers can remain untouched.

My conclusion would be that you should expect SOME pain. There are patients who are more sensitive and need some medication, at least in those first days, while others are “dancing” through the treatment. Just don’t fear this pain that much since it doesn’t last and in some cases it might not even appear. Just schedule that meeting with your orthodontist and DO IT. I tell you I don’t regret this, even if I was in pain for weeks. My treatments were so easy that I would be crazy not to finish this or not think that it’s too bad I didn’t do this earlier.

Only trace amounts of one of the PAHs has been reported to be make known in smokeless tobacco prior to the recent disclosure by Stepanov and colleagues that at least eight PAHs are instant in smokeless tobacco. This decision inspired the new research.

The scientists analyzed the PAHs in 23 moist rappee samples that included various flavors of the most popular brands sold in the U.S. They identified 23 different PAHs in the samples, of which 9 are classed as carcinogens. The technical term for these problems is "malocclusion," which means "bad bite." The practice of orthodontics requires professional skill in the design, application and control of corrective appliances, such as braces, to bring teeth, lips and jaws into proper alignment and to achieve facial balance.

The brace will not mark the teeth. If dental plaque is allowed to become thick around the brace the plaque itself may damage the teeth. Instruction will be given on the use of special brushes. A daily fluoride mouth rinse can be used to harden areas that are very difficult to clean. Care must be taken to choose an alcohol-free, pH-balanced rinse. Certain foods and drinks may also lead to damage. You should avoid drinking lots of fizzy drinks and keep in-between-meal snacks to a minimum.

It is important to realise that once a brace is removed the teeth can drift back towards their original positions.
